CME - Enduring - End-of-Life/Palliative Care CME
This CME course satisfies the State of NJ licensing requirements for 2 hours of End-of-Life Care credits. The method of delivery for this Continuing Medication Education course will be an enduring online 2 hour program. This training session is provided visually by Power Point Presentation and in audio by hearing the speaker’s voice. All other information, such as outcome evaluation and an attestation form, is included in this training course. Participants will need to complete an Attestation Form, an Outcome Assessment and an Evaluation to receive a certificate. Certificates will be emailed out within one week.

Learning Objectives:
- Manage End of Life Symptoms
- Identify End-Stage chronic disease states
- Articulate the philosophy of hospice
- Illustrate delivery of hospice care
- Employ the purpose and function of a POLST form
- Construct a goals of care conversation in patients with advance disease and life limiting illness
- Implement best practices when delivering serious news to patients
- Employ the SPIKES and REMAP techniques in listening and responding to patients during goals of care conversations
- Formulate a treatment plan that matches the patient's goals and values
