  • Classify CKD using CGA and apply KFRE thresholds to referral, multidisciplinary care, and kidney replacement therapy planning.
  • Utilize best practices for interprofessional communications and collaboration for patients with competing cardiac and nephrology care in the clinical and professional environments.
  • Collaboratively manage hypertension-associated atherosclerotic cardiovascular disease and hypertensive heart disease.
  • Recognize how to coordinate care plans for patients with chronic kidney disease across health care settings to improve patient outcomes.
