Learning Objectives:
- Formulate a shared goals-of-care plan for a patient with frailty or advanced dementia that documents one patient priority, one medication-management decision, and a communication plan for the patient, caregivers, and interprofessional team
- Identify the roles of prescribers and nurses in collaboratively identifying high-risk medications and developing a patient-centered deprescribing plan for older adults
- Collaborate and apply an interprofessional approach to the 4Ms by integrating OT findings with medical, nursing, and other team perspectives to develop person-centered plans of care
- Work as an interprofessional team to support patients’ access to goal-concordant care throughout the aging process, whether that goal is independence at home or having care needs met in a more supported living environment
- Recognize the role of additional disciplines and a collaborative healthcare team to help manage and prevent falls in the elderly population
- Identify the roles of the team providing patient-centered care for medical aid in dying and explain eligibility requirements qualification process
